Reach Out Hertfordshire - North Herts Centre for Voluntary Services (NHCVS)

Reach Out is a project that aims to reduce loneliness in those who have recently been discharged from hospital, are are living with frailty by linking them up with a volunteer in their neighbourhood who can offer companionship and practical support to help them to feel less lonely and isolated.

Reach Out allows people to volunteer flexibly to fit in with their own lifestyles and commitments.
